新生健康檢查記錄表

※新生健康檢查注意事項 (須完成以下二點!)
一、【健康資料卡】請進入教務處->學生專區->網路註冊->新生健康檢查->健康資料卡(https://sis.nsysu.edu.tw/ONAMS/apply_index.php?ONA_STR=MEgxNDAwMyxFMjIyNzI0MjY2LEE= )網路線上填寫。請於114年3月21日前務必完成。
二、【健康檢查】依據教育部「學校衛生法」規定辦理，下列二種方式請擇一：
（一）自行前往本校簽約之醫院–健仁醫院:
1. 持「國立中山大學健康檢查記錄表」請雙面列印!並將表單內「重要資訊」及「學生基本資料」填寫完畢帶至醫院受檢。
2. 健檢起訖日期：114 年 1 月 1日起至 114 年 3 月 21 日止。
3. 費用:新台幣600元整。
4. 攜帶身分證或居留證或護照，以利核對身分。
（二）自行於全台公、私立地區醫院完成健檢：持「國立中山大學健康檢查記錄表」請雙面列印!並將表單內「重要資訊」及「學生基本資料」填寫完畢帶至醫院受檢；於114 年 3 月 21 日前繳交或送(寄)至健康中心(活動中心大樓，郵局的旁邊；高雄郵政59-49信箱 健康中心收)。

※Precautions for the health checkup of new students (the following two intructions must be completed!)
1. 【Health Information Card】 Please enter the website , Academic Affairs Office->Student Area->Online Registration->Freshman Health Examination->Health Information Card (https://sis.nsysu.edu.tw/ONAMS/apply_index.php?ONA_STR=MEgxNDAwMyxFMjIyNzI0MjY2LEE= ) Fill in the online form.
Please complete it before March 21, 2015.
2. 【Health Checkup】 It is implemented in accordance with the provisions of the School Health Law of the Ministry of Education. Please choose one of the following two options:
1. Go to the hospital contracted by our school – Jianren Hospital:
(1) Please print both sides of the "National Sun Yat-sen University Health Checkup Record Form"! Complete the "Important Information" and "Basic Student Information" in the form
(2)Fee: NT$600, please bring your own change!
(3)Bring your ID card, ARC or passport to facilitate identity verification.
2. Complete your own health check-up at public and private hospitals across Taiwan: Please print both sides of the "National Sun Yat-sen University Health Checkup Record Form"! Complete the "Important Information" and "Basic Student Information" in the form and bring it to the hospital. Get examined; submit the before March 21 2025 and send it to the Health Service Center (Activity Center Building, next to the post office; Kaohsiung Post Box 59-49 Health Center receive).
