| 一、【健康資料卡】請於115年8月18日有學號以後進入教務處->學生專區->網路註冊->新生健康檢查->健康資料卡(https://sis.nsysu.edu.tw/ONAMS/apply_index.php?ONA_ITEM_NO=0H14003&CTYPE=A )網路線上填寫。 |

| 二、【健康檢查】依據教育部「學校衛生法」規定辦理，下列三種方式請擇一： |
|
| （一）到校健康檢查： |
|
| 1. 115 年 9 月 3、4、5 日(星期四、五、六)上午 8：30至下午 4：30，所有新生均已安排校內健檢時段，請新生盡可能到校健檢，省時省錢又方便! |
|
| 2. 持「國立中山大學健康檢查記錄表」請雙面列印!並將表單內「重要資訊」及「學生基本資料」填寫完畢帶至學校受檢。 |
|
| 3. 費用:新台幣650元整，請自備零錢! |
|
| 4. 攜帶身分證或居留證或護照，以利核對身分。 |
|
| （二）自行前往本校簽約之醫院–健仁醫院: |
|
| 1. 持「國立中山大學健康檢查記錄表」請雙面列印!並將表單內「重要資訊」及「學生基本資料」填寫完畢帶至醫院受檢。 |
|
| 2. 健檢起訖日期：115 年 8 月 12 日起至 115 年 10 月 23 日止。 |
|
| 3. 費用:新台幣650元整。 |
|
| 4. 攜帶身分證或居留證或護照，以利核對身分。 |
|
| （三）自行於全台公、私立地區醫院完成健檢：持「國立中山大學健康檢查記錄表」請雙面列印!並將表單內「重要資訊」及「學生基本資料」填寫完畢帶至醫院受檢；於114 年 10 月 17 日前繳交【限114年1月1日以後之健檢報告】送(寄)至健康中心(活動中心大樓，郵局的旁邊；高雄郵局59-49信箱 健康中心收)。 |

| 1. 【Health Information Card】 Please enter the website after 2026 Year August 18th with student IDs, Academic Affairs Office->Student Area->Online Registration->Freshman Health Examination->Health Information Card (https://sis.nsysu. edu .tw/ONAMS/apply_index.php?ONA_ITEM_NO=0H14003&CTYPE=A ) Fill in the online form. |

| 2. 【Health Checkup】 It is carried out in accordance with the "School Health Act" of the Ministry of Education. Please choose one of the following three methods: |

| (1) Health check-up at school: |
|
| 1. From 8:30 am to 4:30 pm on 2026 Year September 3th, 4th and 5th (Thursday, Friday, and Saturday), all freshmen have been scheduled for on-campus health checkups. Freshmen are requested to come to school for physical checkups as much as possible to save time. Save money and convenience!

| 2. Please print both sides of the "National Sun Yat-sen University Health Checkup Record Form"! Complete the "Important Information" and "Basic Student Information" in the form and bring it to the school for checkup. |
|
| 3. Fee: NT$650, please bring your own change! |
|
| 4. Bring your ID card, ARC or passport to facilitate identity verification. |
|
| (3) Complete your own health check-up at public and private hospitals across Taiwan: Please print both sides of the "National Sun Yat-sen University Health Checkup Record Form"! Complete the "Important Information" and "Basic Student Information" in the form and bring it to the hospital. Get examined; submit the [Health Examination Report Limited to 2026 Year January 1st] before 2026 Year October 23th and send it to the Health Service Center (Activity Center Building, next to the post office; Kaohsiung Post Box 59-49 Health Center receive). |
